He was predeceased by: his parents, John Polk and Jan Polk. He is survived by: his wife Lana Polk; his children, Ashley Ayers (Cody), Noel Ellis (Caleb), Nathan Polk (Kelsey Moore) and Shawna Hutchins (James Cowden); his grandchildren, Charlie, Addy Ayers and Wyatt Ellis; and his siblings, David Polk (Dana), Melody Monella (Tim) and Barron Polk. He is also survived by several nieces and nephews; and many treasured friends.
