She was predeceased by: her parents, Eileen Dehning and Donovan Smith. She is survived by: her husband Jeffrey Ban; her children, Justin Ban (Anna) and Kristin Otts (Joshua); her grandchild Grayson Otts; and her brothers, Donnie, Eddie, Kennan and Chad.

In lieu of flowers, the family ask that donations go to the American Cancer Society in Pam's name.
