He was predeceased by: his son Curtis. He is survived by: his wife Donna; his daughters, Susan Slack (Roy) and Jane Douglas (John); and his grandchildren, Stephanie, Daniel Slack, Joel, Megan and Heather Douglas. He is also survived by 5 great-grandchildren.
