He was predeceased by: his spouse Luigina'Gina'; his parents, Francesco Bevilacqua and Paola Bevilacqua; his parents-in-law, Filippo and Anna Scarpino; and his siblings, Frank Bevilacqua (Ida), Giovanni Bevilacqua (Rosa) and Rosa Lombardo.
He is survived by: his children, Frank Bevilacqua (Lisa) and Anne Marie Salvador (David); his grandchildren, Nicholas, Daniel, Anthony and Michela; his siblings, Josephine'Pina'Piazza, Maria DiBartolo (Charlie) and Peter Bevilacqua (Julie Mai); his siblings-in-law, Stefano Lombardo and Lucia Mancuso (late Tom); and his nephew Frank Mancuso (Jessica). He is also survived by many nieces and nephews.
In memory of Sam, memorial contributions may be made to Niagara Health Foundation, Walker Family Cancer Centre and would be appreciated.
