He was predeceased by: his half-sister April Bulger. He is survived by: his parents, Angela and William Collicutt; his son Bailey; his wife Sheri Sazwan; his stepchildren, Matt and Marissa; and his stepparents, Tony Bulger and Jessica Calatan. He is also survived by numerous aunts, uncles, nieces, nephews, cousins and friends.
In lieu of flowers donations can be made in Chad's name to the Canadian Diabetes Association or the local Animal shelter.
